The University of Minnesota and Blue Cross Blue Shield, Center for Prevention, are partnering to conduct a Statewide survey of childcare providers assessing nutrition and physical activity practices in both childcare centers and family childcare homes. This survey is a follow-up to a 2010 survey conducted in partnership with University of Wisconsin, Madison - but for this 2015 survey it has a Minnesota only focus so the old logo from 2010 is no longer relevant.

The design should have the following

State of Minnesota
Childcare providers, healthy kids, nutrition and physical activity focus
Intended outcome of this survey project is developing momentum for support
An indication that we intend to address policy
that this is a follow up assessment from 2010 to 2015

This logo will be used for

• Online (Website, facebook etc.)
• Print (business cards, letterheads, brochures etc.)

This design should not have this in the entries

The logo does not have to include the names of the partnering organizations, unless the acronyms UMN (university of Minnesota) and BCBS (Blue Cross Blue Shield) can be woven in seamlessly.

The logo does not have to include the full name: 2015 childcare provider assessment, unless it works well.
