My audience is super-busy people. Entrepreneurs, organization leaders, and professionals.

The design should have the following

This will be a short quiz with 3 pages. An intro/data input page. An &#039;About Us&#039; page and, most importantly, a results-page.<br /> <br /> The results-page visually shows how much time a repeated activity adds up over time. It shows the accumulated time over different time spans. How much time will activity consumer over the course of a year, 5 years, 10 years and even 20 years. <br /> <br /> Graphic challenge: Think of an activity as a train you&#039;re riding. The longer you ride it the more <br /> time passes. The report shows a train across the top of the table showing accumulated times. <br /> <br /> I want 3 pages.<br /> Introduction/entry page - when submit is pressed if any errors exist &#039;red text should appear next to errored fields. Errors like &quot;this field is required&quot; or &quot;Please enter a valid bithdate&quot;<br /> For ideas on intro page . See the sampleIntroSite page I uploaded- but instead of a soldier my page should show a business person<br /> Result page - shows several tables indicating increase in time<br /> About Us page<br /> <br /> For an idea of what I want look at this sample site: <br /> http://www.marketingdnatest.com/<br /> I also attached a sample data-input page from that site.<br /> <br /> I have a related program. It&#039;s logo is attached and items you&#039;ll make should work well with it.<br /> You must be willing to answer questions about your work. It should be designed without special components. Standard html, javascript or whatever works without having to buy additional tools.<br /> <br /> <br /> Mock pages have been uploaded. Fields that&#039;ll be updated on the RESULTS-page are high-lighted in yellow(these fields will be populated from a php program). This is ONLY for explanation of this job. It&#039;s not meant to be part of the actual final webpage. <br /> I&#039;m really looking to show the increasing amount of accumulated time that occurs over the years... 1,5,10, 20 and even over the lifetime. If the image of the train and tracks could support that would be great. Ideally, the results page should be so attractive &amp; informative that people want to save it like a poster.<br /> Deliverables include and css stylesheet. html pages, &amp; psd files(capable of being opened with CS3 for artwork created). Please let me know if additional files are needed to complete the job.<br /> Similar font and colors as website mentioned.<br /> <br /> You must be willing to answer questions about your work.
