Who we are:

Connecting patients and healthcare providers

Describe your company and organization and target audience.

The company provides a software solution for gathering survey information and delivering individualized health education materials to low income patients. The software runs on a tablets and android phones, and "talks" to patients who have low literacy levels and helps them complete health surveys and screening in privacy and independently. It then delivers tailored health education to the patient based on their answers to the survey (for example, a depression screener -if say they smoke on the survey - it might offer a video to them on smoking cessation methods and also offer to connect them to local smoking cessation programs in their community, etc). Results of the survey are also wirelessly transmitted to clinician, service program or other group. We are thinking of it as a "wait room of the future" where every patient will have their own tablet and own wait room"health-tainment" as they wait for a visit, and also complete surveys they need...

The design should have the following:

we want a sleek contemporary design, perhaps with something a bit quirky or different so it is memorable. we do not want it to look "medical" - so no red crosses or things that evoke 'emergency room' -

This logo will be used for:

Print (business cards, letterheads, brochures etc.)
Online (website, online advertising, banner ads etc.)
Merchandise (mugs, t-shirts etc.)
Signs (including shops, billboards etc.)
Television/screen
